There's a guide to adding custom teams in the Patching Issues forum. Look for the "CUSTOM TEAM" topic - it's a sticky, so it shouldn't be too difficult to find.

There's a few tutorials (jerseys, logos, etc) listed in our Downloads section that you may find useful as well.
