So, say 'Williams' is in there, and your player's last name is Williams, spelled the same way, the PA announcer will say it (as will the commentary announcers). Now, let's say you have a guy named Williams Jr, who is not in the Last Name tab...
What I did was, took some European name that would never be used, overwrote it with 'Williams Jr', and then copied and pasted the numeric code with Williams into my Williams Jr row, so that the game recognizes the name 'Williams Jr' as 'Williams' and the announcers say that name (they will not say WIlliams Jr since that name is not pre-loaded into the game).
I did the same with first names. So the PA Announcer will often say a first and last name, if they both read properly.
You can use a lot of names that sound similar, or have different spellings, etc.
I did it all with a lot of trial and error, so these are just what I believe to be true, and have experienced. Also worth noting that while the PA Announcer will say first and last names, the commentary announcers will only call players by last name (I believe the Audio ID is what links first and last for them, and those are pre-determined, presumably along with the different stories and anecdotes that they tell about the player).
Ultimately I bring very little to this mod (outside of uniforms and organizing it), so I try and finagle what I can, on the rare occurrence that I have a bright idea (that actually works).

I d/l-ed the first version (MM2k14 I think) and it worked great. I am having a bit of trouble with the new MM Live beta v1 playing any other mode except for an exhibition game. In the association / playoff / season modes i am able to set everything up, but when i go to play a game the whole program closes. Is anyone else running into this? I am running it on steam windows 10. Thanks so much again and keep up the great work!

You don't have to replace a European name either, you can clone any name but one thing you always have to make sure is that the names are alphabetized. The "Williams Jr." name should be moved right after the "Williams" name for it to work, you cannot put the names out of order, or else everything will not work and players with names will be called by their jersey number. We've done this for PBA 2K14 ever since v4.4 (January 19, 2015 Update). Unfortunately, you cannot make the last names play first name audio either.

For example, I got him to say 'Chase Jeter', 'Sam Cassel' (for Cassel Jr -- although I could potentially just have used Sam's AudioID instead), and lots of other first names that are just different spellings or pronounciations of other names in the game (after last night's release I added Derryck as a first name, since they had Derek, Derrick, etc. so now the PA should say Derryck Thornton's full name as well). The commentators won't ever do it, because it would not sound natural to splice together a first and last name from a human voice -- with the PA's long and drawn out announcing, you can't tell. I understand why they do it.
