Hmm, you may have to recreate the user profile on the controller as the main user profile. I am not sure, though. I have read that the user profiles are assigned by what you made them with. If you used the keyboard to create the user profile, it probably assigned it to the keyboard. Just a guess.
Try renaming the file extension of the profile in Windows Explorer (C:\Documents and Settings\User Name\Application Data\NBA 2K9\Saves) and then create a new one with the controller.

Ok...finally figured this out. When the start screen comes, do not click enter with the keyboard, click the corresponding enter button on your joystick and it will make it the active controller and not the keyboard.....Issue solved.

Not really. My gamepad is always reset to some default at the start of the game. I have to press the default start button of my controller, start a quick game, pause the game and from there on my configuration is loaded / my gamepad starts to work as it should.
There is no other way for me but starting a quick game for some seconds to get the buttons to work correctly is not that difficult.

Thats a different issue, the question raised was how to default to the controller vs. keyboard once you start the game, as it causes problems in association mode. Sounds like your keys are not being mapped/saved correctly once you exit the game.
