Auto subs may occur if you are playing someone out of position (i.e., a SG at PG or SF at PF), or if you have on CPU assist and need the edge to win.
For flagrant fouls, move up the foul slider (more sensitive), then use the hand check when guarding a player (or, call the "intentional foul" play). Players with low defensive awareness under these settings are more likely (but not guaranteed)to commit flagrant fouls.
I have personally injured an opposing player (Tim Duncan, no less - broken wrist)while playing a season game, so it can happen.
