Try placing this settings.dat file in the settings folder (found in the NBA Live 2004 folder under your Documents). It'll select the Bulls as your favourite team but you can change that, along with any other settings you choose. Hopefully, it'll get you past that initial setup/selection screen. I'm not sure what else to suggest at this time. You can extract the file using 7-Zip, if you don't already have it installed.
