First off I can run the game perfectly fine at medium settings, so please don't take this as a complaint.

That being said, the only reason I don't push the settings even higher (it'd be nice to activate the special effects) is because I risk framerates of 50-55 fps. I typically couldn't care less if I were getting 55, 60, or even 40, but 2k15 feels unbelievably jerky when it's running at anything other than a constant 60. I shouldn't be able to blatantly tell when the fps has dropped from 60 to 59 or 58, but I can.

Is this a known issue with the game, or do I need to make adjustments in the Nvidia control panel?

I think that jerkiness is drops in the frame rate or even associated with some screen tearing. You want Vsync on, but from my testing the game doesn't like Nvidia's fancy options so don't use Nvidia's Adaptive Vsync (even though it should technically be better).

I select Use the 3D Application Setting (also turning Triple Buffering On) and select it in the 2K game settings.

These settings along with the patch have eliminated all jerkiness on my system.

I actually never really noticed micro stuttering before, or after the patch. When I run at appropriately low settings and get a constant 60 fps (not counting cutscenes or closeups after baskets) the game feels smooth as can be. It's only jerky below 60 fps.

Are you saying the game is running really smooth below 60 fps for you?

try turning off the vsync option if you want see how can your system go, i tried it and my fps went up, but not smoothly, it plays around (78-81) and during cut scenes it drops to 48-55, so It's better on. :/

Exactly, if you turn off vsync and the game struggles to get up to 60(Or your refresh rate) or above, then you need to turn some settings down before you turn vsync back on.

I really don't have any interest on exceeding 60 fps--I can get a constant 60 on medium settings and it looks and feels great. Any higher than that (with vsync off) and I sometimes notice some screen tearing.

What I DO want is to be able to keep my current settings, but also turn the special effects on. The special effects (only way to get any court lighting/reflections) is pretty taxing on my hardware and drops me down to 50-55 fps, which apparently introduces serious stuttering with or without vsync.
