or mhm maybe delete the old game sliders, settings etc? I really don't know what to say to you, cause i always sub-in,sub-out players myself so this bug didn't happen to me.

It happens for both the CPU and yourself.
I guess theres no way too fix it. Maybe you should try turn off fouling out.

I have this problem too,but i play in less minute(4-5)/quarter game play..is this hapens in 12 min/quarter too?
