There are no dunk combinations for 5 on 5 gameplay.

There are no such combinations for normal games. You can only perform those dunks in the Dunk Contest.

Just enable slam dunk school, then pause the game, enter options, then configure controller and there will be all information you need, man. If you can't do that from the pause menu (I'm not sure about that) just do it from the main menu.
