Creating Shoe Patches

I've been trying to make a shoe patch but this tutorial http://wiki.nba-live.com/index.php?n=Tutorials.CreatingShoePatches isn't giving me a whole a lot of info. I know how to get the image into the FSH file and into the game. I just don't know how to make the template. Can someone please help with making this template?

Did you looked at Big-D aka Granados's thread at NBA Live 2005. At last page there was a template...

I just ended up altering an existing template using cut & Paste, gray scale and paint. THe template doesn't look great but the shoes look great in the game.
