5 Fouls in First Quarter, Not Pulled

Is this to do with setting the rotation timeline (advanced rotation)?

CPU v CPU Bulls vs Lakers in the finals - a player has 5 PF in the first quarter. He should have sat with 2, certainly 3, but he's still in the game. Do teams follow advanced rotation even if players are in extreme foul trouble?

I think advanced rotations makes players play their minutes even if the game is a blowout. But not sitting for extreme foul trouble is even worse than that.

Players in foul trouble don't get subbed out until a dead ball. A player with 6 fouls will sometimes continue playing until a dead ball also.

I've not seen a player with 6 fouls not leave the game immediately.

There were multiple stoppages after the 5th foul including the fouls themselves, in between the free throws, and time outs.

If a player gets a 6th foul, that is a dead ball. He would be immediately removed.

The same issue is present in MyCAREER. Even if you pick up a couple of quick fouls, you'll be left in there to play your usual minutes. I have to admit I kind of like it for that mode specifically, since you get to keep playing and you can just be very careful and not take too many risks with steal attempts and so forth. Generally speaking though, the coaching AI should follow the common approach of pulling players who pick up two or three quick fouls.
