I had this same problem. Haven't looked into fixing the generic shoes yet but I was able to get the '04 Wolves logo to work.
What I did was copy all of the logos_size.cdf and logos_size.iff files from the first of Med's three folders into my program files NBA 2K13 folder not the ones from Med's second folder (Replace size with tiny/small/medium/large).
You will note that each of the logos_size files from folder 1 are bigger than the logos_size files from folder 2. In this case, bigger is better as you want to use the files from folder 1.
You also need to make sure you copy all of the logo####.iff files from folder 1 to the NBA 2K13 folder in program files and replace the original files (logo1011.iff is for the capital A logo in the game which will be replaced by the '04 Wolves logo).
The '04 Wolves logo should now work properly in every different menu within the game (Rosters, Rotations, etc.).
As noted above, I have not looked into the generic shoes but off the top of my head I would guess that you may be able to use Leftos' Roster Editor to change the shoe brand for each player, though this may be fairly time consuming.
Maybe Medevenx will have a better solution for you though. I myself have not taken the time to check which players have generic shoes who are not supposed to. It might be helpful to make a list of these players and post it here so everyone knows which players need to be fixed.
Other than that, I hope you get your '04 Wolves logo working and if anyone can provide any help with the generic shoes issue please do so.

I realized that the logo_size files from folder 1 fixed the '04 Wolves logo but then the logos for some of the current NBA teams went back to their old logos (Bobcats logo went back to its old colours and Raptors logo went back to the dinosaur with the word "Raptors" above it instead of their current logo which is just a raptor claw). Therefore, I used the 3DM Mod Tool and replaced the capital letter A logo with the '04 Wolves logo in the logos_small.iff, logos_medium.iff, and logos_large.iff in folder 2 (logos_tiny.iff already had the '04 Wolves logo for some reason so it does not have to be changed). After you edit those three files, you should use all of the logo_size files (cdf and iff) from folder 2 now instead of from folder 1 which I told you to use before. Now, once you paste the files from folder 2 into the NBA 2K13 folder in program files, in the game you should see all current teams having their correct and current logo and the '04 Wolves logo should also work for you now too!!!
